Job responsibilities

CLINICAL

Provide a comprehensive high-quality clinical pharmacy service in order to facilitate safe, effective and cost effective use of medicines, to optimise patient care and to promote effective medicines optimisation.

Contribute effectively as part of the multidisciplinary team, by attending ward rounds or other appropriate meetings, to make pro-active interventions in individual patients therapy and to provide information on drug related issues.

Provide counselling (supported by the provision and development as necessary of specific patient information leaflets) to patients (and/or carers where appropriate) on any aspects of medication use, in order to improve understanding and compliance with the prescribed regimen.

Work alongside members of the anticoagulation clinic team to provide safe and effective anticoagulation.

LEADERSHIP AND MANAGEMENT

Supervise as appropriate, the work of pharmacists undertaking the diploma in clinical pharmacy, in order to facilitate safe, effective and cost-effective use of drugs to optimise patient care.

Supervise trainee pharmacist with their research projects as necessary, in order to assist them in obtaining their professional registration and to ensure that departmental quality and cost improvement targets are met.

EDUCATION & TRAINING

Participate in the teaching and supervision of undergraduate pharmacy students, trainee pharmacists, and postgraduate diploma pharmacists, medical and nursing staff to support them in their induction, development and obtaining their qualification.

Contribute to the multidisciplinary education and training within the rotational area on all aspects of medicines management.

Undertake any training necessary for the safe and efficient performance of duties.

RESEARCH AND DEVELOPMENT

Participate in multidisciplinary and departmental audit/research activities.

Contribute to the Research and Development plan for the Pharmacy department.

PROFESSIONAL

Participate in the general departmental discussions about best practice and be involved in any change management developments.

Professionally screen, dispense and check prescriptions in accordance with professional and local procedures, in order to ensure the safe and cost-effective delivery of medicines to patients

Participate in the weekend, late night and on call rota duties.

Experience

Essential

• Significant post-registration experience of clinical pharmacy service delivery.

Desirable

• Experience of working in a similar role

Skills

Essential

• Accurate and concise written and oral communication skills
• Able to receive, summarise and interpret a wide range of clinical information to a wide range of professionals across the trust.
• Able to work effectively to short, medium, long term and changing deadlines
• Able to review and provide specialised advice on medicine use in specific clinical areas
• Able to work independently and under guidance, within departmental, Trust and statutory and professional codes of practice/ethics
• Computer literate

Knowledge

Essential

• Understanding of Information Governance and Confidentiality
• Broad knowledge of therapeutics
• Knowledge and understanding of the formulation, preparation, and therapeutic action of medicines.

Qualifications

Essential

• MPharm or equivalent
• Registered as a Pharmacist with the General Pharmaceutical Council (GPhC)
• Diploma in Clinical Pharmacy or working towards diploma or equivalent experience

Desirable

• Independent Prescribing qualification
• Working towards Royal College of Pharmacy faculty accreditation
